Script Introduction
This script lets the viewer choose a site or page from a list of radio buttons, and then go
to the chosen location when a form button is clicked. It is set up for 3 choices by default.
Cut and Paste Code: BODY Section
This code should be placed between the <body> and </body> tags.
Installation Instructions

Paste the BODY Section code from above between the <body> and </body> tags on your page,
exactly where you want the forward and back images to appear.

Find the matching lines below in the script and change the text in red to
the URLs and text you wish to use:
